Lutheran Community Services Northwest

Lutheran Community Services Northwest (LCSNW) in partnership with the Chinese Information and Service Center’s (CISC) overall goal in this project was to provide access, services and support to help foreign born elders meet their Social Determinants of Health (SDoH) needs in culturally relevant and appropriate ways while improving access to, and engagement with, the healthcare system.

The program model was built with the ultimate outcomes focused on seeing improvements in enrolled foreign born elders’ physical and mental health when several elements are in place: Community-based partnerships, Coordinated Entry, Peer Navigation, and Social Groups.

LCSNW and CISC worked closely and collaboratively to create a universal intake form designed to assess the needs and wants of foreign born elders using updated SDoH assessment techniques for services that each organization provides.

“We have demonstrated (while creating a proof of concept) that there is a strong need for physical/mental health support among our foreign born elders while simultaneously gaining even greater insight into how these various populations and communities' needs/wants often require specific care, support and resources.”
