Case Management
Partners*

Contracted to provide care coordination services to clients referred through the Community Hub

Health-Related Social Needs (HRSN) Benefit Partners*

Contracted to provide specific social care services funded through the Community Hub

Exchange Partners*

Integrated with Connect2 technology to enable coordinated care for their clients 

Referral Partners

Refer clients to the Hub to receive case management services and/or receive client referrals from case management partners to provide services

Input, Learning & Guidance Spaces

These co-creation and engagement spaces are designed for organizations and individuals to share resources, learn from each other, and help shape the Network’s efforts.

Advisory Group*

The purpose of the Community Care Network Advisory Group (AG) is to form a coalition of organizations, leveraging their knowledge and expertise to co-create, provide guidance to, and coordinate a unified network of health and human services providers. The shared goal is to strengthen care coordination and collaboration through universal access and interoperability between platforms/networks to better meet the holistic needs of clients and patients. This membership group is the evolution of the Connect2 Advisory Group, it meets bimonthly and is led by two community co-chairs.

Network Partner Workgroup

The Community Care Network Partner Workgroup (NPWG) provides input on the development of the Community Care Network, including the Connect2 technology. This group creates space for relationship building, gauging community needs, sharing community resources and care coordination innovations, and discussing needs related to Connect2 Technology. Meetings typically feature at least one organization highlighting their services in addition to input and guidance discussions. This open membership group is facilitated by HealthierHere staff. Hub All Cohort Convenings*

Hub All Cohort convenings occur monthly and serve as an opportunity to support shared learning and provide a convening space for all contracted Hub case management partners and their community-based workforce. All Cohort meetings include receiving programmatic updates, the opportunity for partners to give input, and for CHWs to network and share best practices.

Community-Based Workforce Advisory Committee*

An advisory committee made up exclusively of Community Based Workers from the Community Hub Case Management Partners and supported by HealthierHere staff. This committee's purpose is to foster collaboration among the workforce and to ensure that CBW voice is incorporated into the Community Hub. This space allows Community Health Workers (CHWs) in Case Management partner cohort to share insights, address challenges and offer mutual support along with the chance to explore opportunities for increased collaboration amongst organizations.
