The HealthierHere Community Care Network (CCN) is an innovative social care network focused on improving health equity in King County, WA through whole-person care coordination. The network brings together community, tribal, native-led/native-serving, social service, healthcare, and behavioral health organizations to address people’s unmet social needs that are essential to health and well-being.  

HealthierHere organizes and supports the network, offering organizations and community members many ways to participate and partner. Centered on community voice, co-creation, and the expertise of the community-based workforce, the network fosters, learning and collaboration across sectors to improve health outcomes for all people in King County.

Personalized, community-based care coordination

Through the HealthierHere Community Hub, we leverage the power of person-to-person relationships, matching clients with trusted community guides to help them find culturally and linguistically responsive resources and services.

Cross-sector collaboration and learning

Organizations and individuals can connect to share resources, foster innovation, and build collaborative relationships through workgroups, convenings, and trainings.

Equitable co-design opportunities

Utilizing the strength and expertise of the community-based workforce, we provide forums to co-create solutions tailored to our region and aimed at system transformation.
