Today’s system of health and human services is fragmented and disconnected. It isn’t easy to find the right resources, refer people to services, track if those services were received, or understand who else might be providing care and support.

Connect2 is a suite of technology solutions co-created by HealthierHere and our partners to enable organizations to collaborate and share data more effectively. Connect2 tech powers the HealthierHere Community Hub and Community Care Network. We are also working to support the Hubs of other Accountable Communities of Health across Washington state through Connect2.

HealthierHere supports organizations to adopt or integrate Connect2 technology, providing technical support, tools, and relationship-building opportunities.
