Neighborhood House
Neighborhood House (NH), in partnership with Chief Seattle Club (CSC), provided culturally relevant resource navigation, benefits enrollment, and ongoing care coordination services to Native elders and adults with disabilities in CSC permanent supportive housing and day center programs to help community members reach their health and wellness goals.
CSC has extensive experience supporting unhoused American Indian/Alaska Native (AN/AI) community members in King County, while NH has extensive experience assisting marginalized communities in navigating the aging and disability services system.
Through this partnership, they brought together their shared strengths to increase rates of benefits enrollment, improve health outcomes, and increase housing stability for American Indians/Alaska Natives (AI/AN) in King County.
“In working to provide direct service support, while also collaborating to advocate for systemic changes to improve the cultural responsiveness of long-term services and supports, NH and CSC can shift the community living experience for Native elders and individuals with disabilities in King County now and in the future.”
