
HealthierHere has opened the 2026–2027 Community Hub Case Management Partner Procurement. We are specifically seeking organizations with demonstrated experience serving Veterans in King County and organizations with an established operational presence in North Seattle.
Organizations must have existing care coordination or case management infrastructure in place, including the capacity to hire, train, and supervise Community Based Workers (CBWs), to be eligible.
Who Should Apply
Eligible applicants include community-based organizations (CBOs), tribal serving organizations, and health or behavioral health providers operating in King County, WA that currently provide case management services and align with one or more of the following priority areas:
- Demonstrated experience serving Veterans
- Strong service presence in North Seattle communities
Contract Structure
Contracts are structured using a CBW multiplier of $147,000 per 1.0 CBW FTE, which covers wages, benefits, and supervisory costs. Partners may budget for additional direct costs and an applicable indirect rate. A 5% performance bonus pool is available for organizations meeting contractual benchmarks.

- Completed Hub Partner Interest Survey Form
- Completed Readiness Assessment with supporting documentation. Applicants must score 75% or higher on the Readiness Assessment.
