You must receive your first vaccine dose between December 1, 2022 and February 28, 2023 to be eligible for this reward. This reward is available to Molina Healthcare of Washington - Apple Health (Medicaid) members ages 6 months and older.

You have until March 31, 2023, to submit your vaccine details. Please allow 4-6 weeks to receive the gift card.

This reward is in addition to the $200 yearly reward maximum.
