HealthierHere is excited to share our 2024–2028 Strategic Priorities, a roadmap to eliminate health disparities and ensure all people in King County can achieve their best health and well-being.

Grounded in our mission, vision, and core values of equity, community, partnership, innovation, and results, this plan outlines our commitment to transforming health and social care systems through collaboration, accountability, and community-driven solutions.

Our Strategic Priorities for 2024–2028

In the coming years, HealthierHere will focus on three key priorities:

Coordinated Whole-Person Care

We will strengthen care coordination across health and social care systems so that people, especially those who have been historically marginalized, can access the services they need, when they need them. This includes building sustainable infrastructure, expanding our Community Hub, and growing the use of our Connect2 technology to improve data sharing and care coordination throughout King County and across Washington State.

Equitable Systems Change

To achieve lasting change, we must address the root causes of health disparities. That means centering community and Tribal voices in decision-making, strengthening our partnerships, advancing equitable practices, and ensuring accountability at every level. Together, we can transform systems so that they work better for everyone.

Sustainability and Impact

HealthierHere is committed to ensuring our work and partnerships endure beyond 2028. We will diversify funding, strengthen our 501(c)(3) operations, and develop grantmaking strategies to support the long-term sustainability of our organization and the community partners who make this work possible.
