HCA has been working with WA State health care plans and primary care providers to develop a Multi-payer Primary Care Transformation Model. The model will serve as a framework to strengthen primary care through multi-payer payment reform and care delivery transformation. It aims to create better health and better care for patients, smarter spending, and improved clinical experience for providers.
