Background

HealthierHere is a non-profit regional collaborative committed to reducing health disparities, advancing equity, and improving the well-being of all people in King County, Washington State through innovative, cross-sector collaborations. Over the past three years, HealthierHere has been leading the co-design of the Connect2 Community Network together with 100+ health care and community-based organizations (CBO). The Connect2 Community Network (C2CN) is a care coordination network that will connect people, service organizations, and health care, community, and Tribal partners.
