We are excited to announce that orientation training for our first cohort of Case Management Partners for the HealthierHere Community Hub is now underway!
This first cohort brings together 17 partner organizations serving diverse communities here in King County. We are excited to see the contributions each will make through their work with the HealthierHere Community Hub and are grateful for their commitment to creating a healthier, more equitable community.
What are Case Management Partners?
Case Management Partners serve as partner agencies of the HealthierHere Community Hub, helping community members address health-related social needs through care coordination. Partners are contracted with the Community Hub and dedicate or hire supervisory staff and Community Health Workers (CHWs) to carry out these efforts.
Community Health Workers serve as trusted guides and advocates for clients, drawing on shared lived experiences, languages, and cultures to help them navigate systems and connect to the resources and services they need to thrive. The Hub’s CHWs do client outreach, receive referrals through the Hub, introduce the program and collect client consent and authorization upon enrollment, assess client needs and set goals, connect clients to resources, check in with clients regularly, and discharge clients when appropriate.
What organizations are part of the first Cohort of Case Management Partners?

Here are some unique facts about our first cohort:
- Across the cohort of 17 partners, 29 different languages and dialects are represented.
- Our partners serve all areas of King County, and our first cohort has a particular focus on South King County and Central and South Seattle.
- Around 40% of our partners have staff who can provide services in Spanish.
- Our cohort includes a mix of small, grassroots organizations and larger Community Based Organizations (CBOs).
What training is being provided to Case Management Partners and how will this prepare for a successful Hub launch?
Training is designed to build capacity to enable Case Management Partner agencies to more effectively serve their communities through the Community Hub.
Throughout the rest of July and August 2024, Case Management partners will be participating in a four day, eight-session comprehensive training that covers case management skills, navigating and documenting in the Connect2 Coordinator (a Client Management System co-created by HealthierHere), and the Community Hub workflow. Additional training will be provided to partner staff following the orientation as we build a community of practice.

“We’re excited for the launch [of the HealthierHere Community Hub] to learn more about some additional ways that we’ll be able to assist folks that come through the pipeline for assistance,” said Sciullo. “We’re going to be slightly expanding our team of case managers at the Y and bring some more folks on into this work, so we’re really excited to see what comes of it.”

To self-refer or refer a client into the HealthierHere Community Hub for Case Management services, please use the online intake form. When referring others to the Community Hub, or if you have been referred by a specific organization, please select that organization from the drop-down list on question 1 of the online form.
